New Patient Registration
Our practice area consists of SE3, SE7, SE10 (core), SE8, SE9, SE12 and SE18 & SE28 (outer boundary) and patients who live in these areas may register with the practice.
It is important that a new registration form is completed before you actually need to see a doctor or nurse, if possible, to ensure that we have your previous medical history and that we can carry out a “New Patient Screening” to ensure that you get the best possible treatment.
The relevant forms are available at reception or can be downloaded from the above "Registering Online" tab.
Medical treatment is available from the date of registration. Please contact reception for further information.
Please note that all new patients are required to have a health check up within 6 months of registering to meet the NHS guidelines. with the GP or Practice Nurse.
New Patients at registration can request On Line access to appointments, repeat prescriptions and medical summary. Please note that Photo ID is required when requesting this service.
Changes to Personal Details
Please let our receptionists know of any change of name, address or telephone number as soon as possible so that we can update your records accordingly.
